How normal cells become brain cancers

ScienceDaily (Sep. 28, 2011) ? Brain tumor specimens taken from neurosurgery cases at the University of California, San Francisco (UCSF) Medical Center have given scientists a new window on the transformation that occurs as healthy brain cells begin to form tumors.

The work may help identify new drugs to target oligodendroglioma, a common type of brain tumor, at its earliest stage, when it is generally most treatable. Any potential drugs identified will have to prove safe and effective in clinical trials, a process that can take several years.

As described in the journal Cancer Cell this month, the UCSF team found that the pool of cells from which oligodendroglioma tumors emerge normally divide "asymmetrically" by splitting into two unequal parts -- like giving birth to fraternal twins who look different and have distinct fates. When these normal cells transform into cancer cells, they switch gears and begin dividing symmetrically, essentially giving birth to identical twins instead.

"This happens early -- before the tumor forms, and it may provide a point to intervene in the process of tumor initiation," said Claudia Petritsch, PhD, an assistant professor with the UCSF Brain Tumor Research Center who led the research.

The Brain Tumor Research Center is part of the UCSF Department of Neurological Surgery, which is consistently ranked by U.S. News & World Report as one of the top departments in the world. Its doctors perform more than 1,100 neurosurgeries a year to remove brain tumors, and in the last 30 years, this work has helped to build one of the most extensive brain tumor repositories in the United States, with tissue samples collected from more than 7,800 cases of cancer.

In their research, Petritsch and her colleagues used genetically engineered mice to identify that a protein called NG2 controls this switch, and they are working on ways to target genes that regulate the process as a way of fighting oligodendroglioma and perhaps other brain tumors.

Why Divisions Matter to Cancer

Oligodendrogliomas are unusual among brain tumors because they often respond to chemotherapy drugs. However, the tumor frequently returns in a form that is resistant to chemotherapy and requires repeated surgical removal.

Petritsch and her colleagues first discovered last year that oligodendroglioma tumors derive from a type of progenitor cell called "oligodendrocyte progenitors" that proliferate in the brain throughout life. These progenitors may also play an important role when the brain is injured by multiplying rapidly and helping heal wounds.

The new studies in mice suggest that lacking an injury, these progenitors divide mostly asymmetrically, maintaining an equilibrium of these cells within the brain. Progenitors can also switch gears and divide symmetrically instead. Scientists believe that allows the brain to provide an expanded pool of cells when needed.

Using mouse models of the tumors as well as tissue samples taken from people with the disease, Petritsch and her colleagues showed that before the tumors arise, the cells preemptively make this switch, transforming from dividing asymmetrically to dividing symmetrically.

They used bioinformatics to discover that dozens of regulators of asymmetric cell division including NG2 are dysregulated in oligodendrogliomas. The Petritsch lab calls these "asymmetry proteins" and argues that if mutated they probably cause the switch to abnormal cell divisions and thereby initiate the genesis of tumors. Modulating NG2 and dysregulated asymmetry proteins pharmacologically may restore normal division modes and provide a new way to fight the cancer with drugs.

Thousands entering Calif. schools without vaccines (AP)

SACRAMENTO, Calif. ? Last year's class of California kindergartners had a record high percentage of parents who used a personal belief exemption to avoid immunization requirements, a development that concerns state health officials.

More than 11,000 kindergartners missed at least one vaccine in 2010 because their parents decided to forgo inoculation. At nearly 2.5 percent of the state's 470,000 kindergartners, that's California's highest rate of declined vaccines since at least 1978, the year before the measles, mumps and rubella vaccine was required.

The percentage is more than double that in certain parts of the state, particularly in more affluent coastal communities in Northern California.

The public debate over childhood vaccinations has been growing throughout California, where last year a deadly spike in whooping cough cases killed 10 babies and sickened more than 9,100 people. The outbreak prompted a state law that requires middle and high school students to get whooping cough booster shots before going back to school this year.

The percentage of parents who sign vaccine exemptions based on personal beliefs has been rising steadily since 2004. The increase coincides with rising use of the Internet for information, said John Talarico, chief of the immunization branch for the California Department of Public Health.

"We really think a lot of it is due to honest, valid concern that parents do the best thing for their child coupled with misinformation that gets out through various forms of communication," he said.

He said state health officials want to study the personal-belief exemptions to better understand trends and behaviors. For now, he is hoping the trend will begin to slow, especially with media coverage of last year's whooping cough deaths.

Just last week, state health officials said the number of reported measles cases in California had reached a 10-year high of 28. Of those, 22 people were either unvaccinated or their vaccination status was unknown.

"When people can see disease around them, it generally drives them to think about the benefit the vaccine can give their children versus whatever else they hear," he said.

Vaccine statistics for this year's kindergartners will not be available until 2012.

Parents can file two types of vaccine exemptions ? a medical exemption or a personal belief exemption. The medical exemption is rarer and typically is reserved for children who cannot be vaccinated because of auto-immune disorders or allergies. It requires a doctor's signature.

For a personal belief exemption, parents are not required to supply any information to explain their decision.

Doctors and medical experts say vaccines are a reliable means of preventing illness with little risk of injury, but some parents don't buy into the safety of immunizations. They cite concerns about vaccines making their children susceptible to autism or diabetes.

In a comprehensive safety review of vaccines issued last month, the Institute of Medicine found there is no link between vaccines and autism or diabetes. The institute, part of the National Academy of Sciences, found that serious side effects of vaccines are rare and can include fever-caused seizures and occasional brain inflammation.

The increasing number of kindergartners entering school without immunizations poses a risk to others, especially children who have legitimate medical exemptions that prevent them from getting their shots, said Linda Davis-Alldritt, a school nurse consultant at the California Department of Education.

"Disease prevention is really a very important thing," she said. "These are diseases that can be very serious, and it can cause death and it can cause long lasting illnesses."

Parents receive information from schools and the Department of Education about the importance of inoculation and the dangers of unvaccinated children spreading infectious diseases to the rest of the community.

Overall immunity of a population to illness typically is achieved when 90 percent of the population is properly immunized. But Talarico, California's top immunization officer, said that can be misleading because unvaccinated children tend to cluster in pockets where like-minded parents decide to forgo immunizations.

"When we see these clusters, that represents the possibility of transmission of disease more quickly and in a more sustained fashion," he said.

A cluster of unimmunized children in San Diego led to an outbreak of measles in 2008 that infected 12 children. Nine of them had not been inoculated because of their parents' objections, while three others were too young to be immunized.

In some schools, as many as 30 percent of kindergartners are vulnerable to at least one vaccine-preventable communicable disease, according to data from the state Department of Public Health. The majority of schools have 100 percent immunization rates.

Certain areas of California have higher exemption rates than others. In Marin, Sonoma and Santa Cruz counties, more than 6 percent of incoming kindergartners in 2010 had parents file a personal belief exemption. By comparison, Los Angeles, San Francisco and Fresno counties had rates below 2 percent.

Barbara Lowe Fisher, co-founder and president of the National Vaccine Information Center in Vienna, Va., said parents sometimes decide to file the personal belief exemption form because they cannot find a doctor who will sign a medical exemption form.

"We've had families whose children have had reactions to vaccines, and some of them have become injured or even died. They want to make an independent, informed decision for their other children, and they can't find doctors who will write medical exemptions," she said.

She said the mission of her nonprofit is to prevent vaccine-related injuries or death through public education. It does not take a position on whether parents should have their children vaccinated, but it does defend a parent's right to opt out of a vaccine.

Some parents base their decision on what they hear from others or see on the Internet, rather than in consultation with a medical expert, said Catherine Flores Martin, director at the California Immunization Coalition, a Sacramento nonprofit that receives some of its funding from vaccine manufacturers.

"Parents have access to so much information on the Internet and I think a parent, even when they're really well educated, will have a hard time sifting through the credible resources versus the anecdotal stories," she said. "You can find whatever you want on the Internet to support your belief."

Wambach leads US

PORTLAND, Ore. - Abby Wambach made the most of her last match before a little rest and relaxation.

The veteran on the U.S. women's national team scored two goals in a 3-0 victory over Canada in a friendly at Jeld-Wen Field on Thursday night.

The exhibition was the second of a two-match Celebration Series to thank stateside fans for their support of the U.S. women's team during the thrilling run in the Women's World Cup in Germany this summer. The U.S. women faced Japan in the final but ultimately lost on penalty kicks.

After Thursday night's match, both the United States and Canada were going to take some time off before preparations for Olympic qualification begin.

"This is the last game for us for the season, so we wanted to go out with a win," Wambach said. "We wanted to give the fans something to cheer about."

The teams played to a 1-all draw in the opening match in Kansas City this past Saturday. Wambach converted an early penalty kick to give the United States the lead until Melissa Tancredi tied it late in the first half. The result ended Canada's seven-match losing streak to the U.S. women.

U.S. coach Pia Sundhage has experimented during the series with a one-forward system, playing Wambach up front.

Canada, meanwhile, has been adjusting to new coach Jon Herdmann after a disappointing World Cup. The sixth-ranked Canadians lost to Germany, France and Nigeria to exit after the first round.

The two teams will start training again in November and December. The top two teams from the CONCACAF qualifying tournament, scheduled for Jan. 19-29 in Vancouver, British Columbia, advance to the London Games. The U.S. will compete along with Canada, Mexico, three teams from the Caribbean and two teams from Central America.

Hope Solo wore the captain's armband on Thursday night to commemorate her 100th cap, earned in the World Cup semifinal against France.

Solo, who is competing on "Dancing with the Stars," was a fan favorite at Jeld-Wen Field, home of the Portland Timbers. She acknowledged the support with a wave when she was subbed out in the second half for Jill Loyden.

But the crowd went wild for Megan Rapinoe, who played at the University of Portland. Twice in the first half she leveled what looked like winning strikes on the net, and twice she hit the crossbar. After the second, she threw her hands up in frustration.

"That's football, I guess," Rapinoe said afterward. "We got the goals in the end."

Wambach saved the day for the United States. She scored an unassisted goal in the 63rd minute and added another goal from out front in the 70th minute. That gave her 125 career goals in international competition. Fans implored for one more by chanting: "Wambach Hat Trick!"

Alex Morgan added the final goal in the 92nd minute.

A crowd of 18, 570 attended the match in Portland, which proclaimed itself Soccer City, USA, back in the mid-1970s when it embraced the Timbers' early success in the North American Soccer League. The city's support of the game has grown throughout the years with the Portland Pilots' success, and currently with the Timbers' leap to Major League Soccer this season.

The Portland Pilots women's team won NCAA titles in 2002 and 2005.

In addition to Rapinoe, U.S. defender Stephanie Cox also played for the Pilots, as did Canadian midfielder Sophie Schmidt. Christine Sinclair, a star on the Canadian national team, did not play in the exhibition series.

"They have such phenomenal players," Schmidt said of the U.S. team, "that sometimes they just have a little magic."

Herdmann said he's happy with what he's seen in his short time with Canada.

"The biggest thing is we have a good foundation and good people to work with," he said.

It's been a rough 18 months for the UFC's building blocks. Chuck Liddell and Randy Couture are now retired after suffering bad knockouts in their final fights. Now Matt Hughes may have to join them after falling badly against Josh Koscheck as the victim of a first-round knockout at UFC 135 in Denver.

Just Liddell like did in his final fight against Rich Franklin, Hughes was more than holding his own against the younger Koscheck. But when older fighters get popped with a good shot, they can't take it like they used to.

Hughes landed some good shots and targeted Koscheck's right eye with some good jabs, but everything turned when he absorbed some nasty right hands in the final minute of the round. Hughes hit the floor in the final seconds. Koscheck jumped on top and poured it on. He landed two big hammer fists with five seconds left and Hughes went limp.

UFC 135: Did Koscheck end Hughes? career with first-round TKO?Hughes wouldn't commit to retiring, but appeared both emotional and really frustrated during his postfight conversation with UFC analyst Joe Rogan.

"I'm not retiring," Hughes said. "I'm going to tell the UFC to put me up on shelf and we'll see what happens after that."

This makes it two straight first-round KO or TKO losses for Hughes, the UFC Hall of Famer and two-time former champ. He lost to B.J. Penn in just 21 seconds at UFC 123.

Koscheck (16-5, 14-5 UFC) is one of the bigger villains in the sport, so Hughes got a huge ovation from the fans at the Pepsi Center. If this was his last fight, he got a great sendoff in his record 26th appearance in the Octagon.

"I loved the crowd support when I came out," said Hughes.

Koscheck talked a lot of trash before the fight claiming that Hughes had been ducking him for years, but he was nothing but respectful after the victory.

"Thanks to Matt Hughes for taking this fight late in his career. He's a legend. I'm really happy that I had the opportunity to fight a legend like Matt Hughes," said Koscheck.

Hughes was the UFC welterweight champ on two separate occasions from 2001-06, piling up a 12-1 record during that period.

This was a must win for Koscheck, since he had everything to lose. Koscheck dropped a title shot against Georges St-Pierre, the current UFC 170-pound champion back in April. He had the orbital bone around his right eye broken. Koscheck went on the shelf for three months and had to limit all physical activity. He was targeting Nov. 19 for his return when he got the call less than three weeks ago to fill in for Diego Sanchez against Hughes on this card.

"It was actually nice being on shelf. I got to focus on other areas. I'm growing as a person and as a fighter," said Koscheck.

Radiation boost for artificial joints

ScienceDaily (Sep. 22, 2011) ? A blast of gamma radiation could toughen up plastic prosthetic joints to make them strong enough to last for years, according to researchers in China writing in the current issue of the International Journal of Biomedical Engineering and Technology.

Whole joint replacement, such as hip and knee replacement, commonly use stainless steel, titanium alloys or ceramics to replace the damaged or diseased bone of the joint. Non-stick polymer or nylon is usually used to coat the artificial joint to simulate the cartilage. However, none of these materials are ideal as they produce debris within the body as the joint is used, which leads to inflammation, pain and other problems.

Now, Maoquan Xue of the Changzhou Institute of Light Industry Technology, has investigated the effect of adding ceramic particles and fibers to two experimental materials for coating prosthetic joints, UHMWPE (ultra-high-molecular-weight polyethylene) and PEEK (polyether ether ketone). Alone neither UHMWPE nor PEEK is suitable as a prosthetic cartilage materials because both crack and fracture with the kind of everyday stresses that a hip or knee joint would exert on them. The problem is that the long polymer chains within the material can readily propagate applied forces causing tiny fractures to grow quickly and the material to fail.

Xue has now demonstrated that by adding ceramic particles to the polymers and then blasting the composite with a short burst of gamma-radiation it is possible to break the main polymer chains without disrupting the overall structure of the artificial cartilage. There is then no way for microscopic fractures to be propagated throughout the material because there are no long stretches of polymer to carry the force from one point to the next. The resulting treated material is thus much tougher than the polymer alone and will not produce the problematic debris within a joint that might otherwise lead to inflammation and pain for the patient.

Xue adds that the treated composite materials might also be more biocompatible and so less likely to be rejected by the patient's immune system on implantation. He suggests that the particular structure of the composites would also be receptive to addition of bone-generating cells, osteocytes or stem cells, that could help a prosthetic joint be incorporated more naturally into the body.

PHOENIX - Kirk Gibson sauntered out onto the field in his familiar, half-hunched gait, stopping at a spot on the grass just outside the third base bag.

With hundreds of fans still lingering long after the final out, Gibson pumped his fist as they pumped him up with a chant of "Gibby!"

Known as a gritty winner as a player, Gibson had guided Arizona Diamondbacks to a division title in his first full season as manager.

Yes, this was sweet.

Playing with the never-give-an-inch intensity of their manager, the Diamondbacks completed an improbable run to their first NL West crown in four years, beating San Francisco 3-1 Friday night when rookie Paul Goldschmidt lined a two-run triple off Sergio Romo in the eighth inning.

"I don't care if it's your first year or your 10th year, it's what you set out to do," Gibson said.

The Diamondbacks completed their worst-to-first turnaround in raucous fashion, rallying to beat the Giants with Chase Field buzzing as it rarely did over the past couple of seasons.

It's no surprise, really.

Arizona has been coming from behind all season, whether it was from early-season predictions of another last-place finish or down a run in the late innings.

And, really, to do it again in the clinching game was the only fitting way for the Diamondbacks close it out.

Ryan Roberts got the go-ahead burst started with a double off Matt Cain (12-11) and Goldschmidt capped it with his first career triple to the corner in right.

David Hernandez (5-3), as he has all season, did his job in the setup role, striking out two after allowing a one-out single in the eighth. J.J. Putz, the closer and set-the-example veteran in the clubhouse, finished it off with his 44th save, sending the Diamondbacks pouring onto the field and the fans into an uproar.

A year after going 65-97, Arizona is 91-66 and heading to the playoffs.

"These are the moments you live for," said Diamondbacks center fielder Chris Young, who squeezed the last out.

The Giants are still alive, but just barely.

With the loss, the defending World Series champions are five games behind Atlanta for the NL wild-card spot with just five games left despite Orlando Cabrera's first homer in 124 at-bats since being traded from Cleveland.

"We are breathing. Five games left, we are still breathing," Giants manager Bruce Bochy said. "That is why there is no need to talk. We've still got baseball."

Behind Gibson's attention-to-detail approach and with a roster of players who seemed to take turns coming through in the clutch, Arizona celebrated its first division crown since 2007 by mobbing Putz near the plate. Fireworks shot out of the scoreboard after Young pulled in the final out and the Diamondbacks, after hugging and shaking hands on the field, stopped to acknowledge the fans from just above the dugout.

Champagne flowed inside the clubhouse and out, with a handful of players returning to shower their families, friends, a few people in the front row. Some of the Diamondbacks even went for a celebratory dip, splashing around the only-in-Arizona pool behind the wall in right-center after rounding the stands for high-fives with their fans.

"This is unbelievable," Roberts said.

It sure was.

Predicted to be another also-ran after a pair of 90-loss seasons, Arizona started the season on a downturn, already 6? games out of first by the end of April.

Then something clicked.

Arizona went through a magnificent May, closing the month 15-3 to take the NL West lead, and stayed near the top through July. The Diamondbacks seized control of the division at the end of August into September, going up by as much as 9? games before San Francisco came charging back with an eight-game winning streak.

But after winning three of four to start this week, Arizona found itself on the cusp of its first NL West title since 2007, needing to beat the Giants just once in the weekend series to pop the corks.

As they have all season, the Diamondbacks had to rally to get the clincher.

Arizona starter Joe Saunders was hit hard and often, but gave up just one run ? on Cabrera's solo homer in the fifth ? on nine hits in seven innings to keep Arizona close.

Cain was 6-1 with a 2.83 ERA his previous eight starts against the Diamondbacks and again had them baffled early, getting them to swing over his big-breaking curveball or pound it into the ground.

Playing with the same fighting spirit they have all season, Arizona finally got to Cain in the seventh inning, when the right-hander walked Goldschmidt with one out and Young followed with a line-drive double to the wall in left-center to tie the game at 1-all.

"They are a team that really does play until the end," said Cain, who allowed three runs on five hits in 7 1-3 innings. "They find different guys each night to spark plug the offense."

This time it was Goldschmidt, who started the season in Double-A.

With Roberts on third and Justin Upton on first after an intentional walk, Goldschmidt took and inside-out swing and sent the ball rattling around in the right-field corner. Roberts scored easily and Upton made it around as the ball clambered around off the odd angles.

Chase Field erupted in a roar after Goldschmidt's hit, which was only the prelude to the celebration that would come later ? one Arizona fans had been waiting four long years for.

"This is what I was hoping to do when I woke up this morning," Putz said.

He and about 42,000 other people.

Over the past year we have been receiving a steady stream of inquiries from readers, agents and property owners about just what is the legal rental tax liability for owners who use an offshore holding company.

We spoke to the firm Phuket office of Duensing and Kippen who are tax and legal advisors who have given the following explanation -

"The use of off-shore entities, such as a company incorporated in the British Virgin Islands ("BVI") to own real estate is not uncommon in Thailand. One reason often cited by investors using such entities organized under foreign law for such purpose is that they believe it will result in significant tax savings. Whereas this may be true with regard to any tax applicable to the "sale" of the real estate by way of sale of the off-shore entity itself, it may not be true with regard to other potential tax liabilities. This article will analyze the tax consequences of an off-shore entity renting out real estate it owns in Thailand.

In general, juristic persons "carrying on business in Thailand" are subject to corporate income tax ("CIT") at the basic rate of 30%. CIT is applicable whether these entitles are incorporated under Thai law or some other foreign law. Entities organized under Thai law are subject to CIT on their worldwide income. However, entities organized under a foreign law are only subject to CIT if such income results from "carrying on business in Thailand".

If an entity incorporated under foreign law that is not "carrying on business in Thailand", but receives payment of assessable income from or within Thailand, such payment is subject to a withholding tax ("WHT") at a rate 10% or 15% depending on the type of payment, for example as follows:

Interest 15%

Royalties 15%

Dividends 10%

Rent 15%

Liberal professions 15%

Where the recipient of assessable income is an off-shore party, the Revenue Code of Thailand ("RC") places the liability for filing the applicable WHT return and submitting the tax payment itself on the payer of the assessable income; in our rental income example, the lessee of the real estate owned by the off-shore entity.

Thus, tax liability for such rental income depends on whether or not the off-shore entity is deemed to be "carrying on business in Thailand". The RC does not define "carrying on business in Thailand". Section 66, paragraph 2 of the RC states that juristic companies or partnerships organized under foreign laws and carrying on business in Thailand are subject to the same tax as those organized under Thai law, but only with respect to income arising from or in consequence of the business carried on in Thailand. In addition, Section 76bis of the RC states:

"For a company or juristic partnership incorporated under foreign laws which has an employee, an agent or a go-between for carrying on business in Thailand and as a result receives income or profits in Thailand, such company or juristic partnership shall be deemed to be carrying on business in Thailand and the person who acts as an employee, an agent or a go-between for the business, whether he is an individual or a juristic person, shall be deemed to be representative of the company or juristic partnership incorporated under foreign laws and shall have the duty and liability to file a tax return and tax payment in accordance with the provisions of this Part, with respect to only the abovementioned income or profits."

The practical implication of Section 76bis is obvious. If the real estate owned by a foreign entity is leased out through an on-shore agent, rental pool, etc., such agent or rental pool company could be deemed to be an "employee, an agent or a go-between" of such foreign entity. The result would be that the foreign entity would then be treated like a Thai entity with regard to the income derived from its real estate located in Thailand and its rental income would be subject to the same tax regime. Furthermore, the on-shore liability to file the tax return and pay the applicable tax on behalf of the off-shore entity would be on the said agent or rental-pool company.

It should be noted, however, that there are exceptions to the above. Depending on the jurisdiction in which such off-shore entity is registered such entity, and thereby any purported on-shore agent of the off-shore entity, may be able to invoke the provisions of a Double Taxation Agreement ("DTA") between the jurisdiction in which the off-shore entity was incorporated and Thailand, thus limiting the potential and scope of exposure to full Thai CIT on such rental income. Unfortunately, however, investors using off-shore entities from jurisdictions that do not have a DTA with Thailand such as the BVI, will not be entitled to invoke the protection. Both their off-shore entity and its on-shore agent have a significantly heightened exposure to full Thai CIT liability on rental income earned in Thailand."

Study: Dialysis 3 times weekly might not be enough (AP)

LOS ANGELES ? A major study challenges the way diabetics and others with failing kidneys have been treated for half a century, finding that three-times-a-week dialysis to cleanse the blood of toxins may not be enough.

Deaths, heart attacks and hospitalizations were much higher on the day after the two-day interval between treatments each week than at other times, the federally funded study found.

The president of the National Kidney Foundation said she was "very troubled" by the results published in Thursday's New England Journal of Medicine.

"We could be doing a better job for our dialysis patients" and that might mean doing it more often, said Dr. Lynda Szczech, a Duke University kidney specialist who had no role in the study.

Kidneys rid the body of waste and fluids. Most of the 400,000 Americans with failing kidneys stay alive by getting their blood purified by a machine three days a week at dialysis clinics ? usually on Mondays, Wednesdays and Fridays or on Tuesdays, Thursdays and Saturdays. In both cases, there's a two-day break between the last session of the week and the next one.

The three-day dialysis schedule has been around since the mid-1960s and gives patients a weekend break from the grueling hours of being hooked up to a machine.

However, doctors have suspected that the two-day hiatus between treatments was risky, and smaller studies have found more heart-related deaths on the day after the gap.

"All the fluids and toxins are built up to the highest extent on Monday morning right before dialysis," said Dr. Anthony Bleyer of Wake Forest Baptist Medical Center in North Carolina, who has done similar studies.

The latest research, funded by the National Institutes of Health, is the largest yet. It was done by Dr. Robert Foley of the University of Minnesota and colleagues. All reported receiving fees from dialysis clinics and suppliers.

The team analyzed medical records of 32,000 people who had in-center dialysis three times a week from 2005 through 2008. The average age was 62 and a quarter had been on dialysis for a year or less. After about two years of follow-up, 41 percent had died, including 17 percent from heart-related causes.

Monday was the riskiest day for people on a Monday-Wednesday-Friday schedule. For those on a Tuesday-Thursday-Saturday schedule, the riskiest day was Tuesday.

Researchers found a 22 percent higher risk of death on the day after a long break compared with other days of the week. Put another way: For every 100 people on dialysis for a year, 22 would die on the day after the long interval versus 18 on other days.

Hospital admissions for stroke and heart-related problems more than doubled on the day after a long break than on other days ? 44 versus 20 for every 100 people treated.

Fixing this problem, however, could be daunting for patients, busy dialysis centers and insurers and it would require a rethinking of how dialysis is currently delivered.

Medicare covers the cost of dialysis, regardless of age, spending about $77,000 annually per person. It covers thrice-weekly treatment, but people can get a fourth session if needed.

Dr. Paul Eggers of the National Institute of Diabetes and Digestive and Kidney Diseases said adjusting how dialysis is done "would require some fairly convincing evidence. I'm not sure this one study would be sufficient to change" standard practice.

Kidney expert Dr. Eli Friedman of SUNY Downstate Medical Center in New York, said he's in favor of every-other-day dialysis or even daily dialysis. But it would mean "a multibillion dollar change," said Friedman, who launched the country's first federally funded dialysis center.

A clinic operator said increasing treatments would require additional staff. And patients also would have to be willing to come in more often.

"They don't even like coming in three times a week. It's completely understandable. It's not fun," said Dr. Allen Nissenson, chief medical officer at DaVita, which runs more than 1,600 clinics around the country.

There has been recent interest in more frequent dialysis after studies hinted that it made people feel healthier.

This year, Medicare started giving clinics a financial incentive to teach patients to do dialysis at home, allowing them to cleanse their blood more often. But this option is not for everyone. It requires intense training and patients need a helper at home.

Unless rules change, Wake Forest's Bleyer said people can take simple steps to reduce their risk by not drinking too much fluid between long dialysis breaks and eating a healthy diet.

"Patients must be a little more careful on the weekend than on other days of the week," he said.

Carol Thomas, who has been on dialysis since 2007, watches her water intake especially on weekends and avoids dairy, beans and nuts, which are high in certain nutrients that can cause complications.

Thomas, of Sacramento, Calif., said home dialysis is not an option because she doesn't have someone to help her. Would she make the trip for dialysis more often if given the choice?

"It's an inconvenience, but probably if it meant lengthening my life," the 69-year-old said.
